2008 Remittance Form
This form must be completed in order for payment to be processed. Submit this form along with all original “FUNDRAISING” stamped receipts to the remittance address below.
NOTE:
- Cheques cannot be processed to individuals.
- Ensure that you have made a copy of all receipts and forms for your records.
- Ensure that your Group Account Number (found on your Registration Form) is filled in the box provided.
- We must receive this Remittance Form together with your original “FUNDRAISING” stamped receipts sent via Registered mail no longer than two (2) weeks after your scheduled shopping week. (Photocopies will not be accepted.) Do not staple or tape receipts.
